Identification of the Dimensions and Components of Culture-Building Based on Citizenship Behavior in Organ Donation

This study aims to identify and conceptualize the dimensions and components of culture-building based on citizenship behavior in the organ donation system through an integrated cultural, social, and psychological framework. This applied-developmental study employed a mixed-methods design. In the qualitative phase, thematic analysis was conducted on scientific documents and 16 in-depth expert interviews drawn from medicine, psychology, sociology, media, religion, education, and transplant coordination, yielding 320 initial codes that were organized into seven core themes. In the quantitative phase, the extracted framework was validated using the fuzzy Delphi technique with participation of 14 domain experts. Data were collected using a five-point fuzzy scale, processed through fuzzy aggregation, defuzzification, and threshold testing to finalize the model components. The findings indicate that culture-building based on citizenship behavior in organ donation comprises seven fundamental dimensions: cultural investment, social investment, psychological investment, development of cultural tools, effective content production, digital citizenship behavior, and institutionalized satisfaction, encompassing 41 validated components. Together, these dimensions constitute a coherent structural model capable of strengthening public trust, reducing cultural resistance, and enhancing civic participation in organ donation. The proposed model demonstrates that sustainable promotion of organ donation requires structural integration of cultural policy, social capital development, psychological empowerment, transparent governance, and strategic utilization of digital communication, offering a robust foundation for national culture-building strategies in public health.
